Please read the following policies and click agree before submitting your form.

A NONREFUNDABLE deposit is required to book your appointment. You will be sent an invoice via email to pay your deposit via card. You have 48 hours to pay your deposit - if not paid within 48 hours, your appointment slot will no longer be held for you. Your deposit will be deducted from the total cost of your tattoo at your appointment. A DEPOSIT WILL BE VOID/LOST IF: - You do not show up to your scheduled appointment. - You are more than 30 minutes late without warning. - You cancel/reschedule with less than 72 hours notice. - After you reschedule 2 times. - You change your desired tattoo concept after booking. - If you decide to cancel your appointment altogether, your deposit is NON-REFUNDABLE. You must be 18 years or older and bring a valid I.D. to your appointment and fill out a consent and release form before getting tattooed. You will prepare yourself for your appointment beforehand by drinking plenty of water, moisturizing daily, limiting caffeine and alcohol consumption the night before, and eating a meal before your tattoo appointment. Forms of payment accepted: - CASH is always preferred. - if you will be paying via card, there will be a card processing fee and tax added to your total. - forms of payment NOT accepted: Venmo, PayPal, CashApp, Apple Pay Aftercare instructions will be provided at tattoo appointment as instructions may vary depending on client/project. Lindsey works TUESDAY-FRIDAY 12pm-7pm and books appointments for 12pm or 4pm. She does not work Saturday, Sunday, Monday. By completing and submitting this form, you HAVE READ and agree to Lindsey's policies regarding scheduling, payment, aftercare, etc.
